Purpose. To learn the features of pathogeny of uterine fibroids fibroids.
Materials and methods. For research selected 122 patients of reproductive age from slow-growing and by a fast-growing uterine fibroids. Prospeсtive research is 80 women with subserosal and intramural localization of myomatous nodes, conduct of pipelle-biopsy of endometrium. The results of pipelle resulted in the selection of two separate clinical groups on the receptor component of uterine fibroids (expression of estrogen and progesterone). Depending on expression of steroid receptors of uterine fiibroids, appointed conservative therapy in two investigated groups under control Ultrasonic research in a dynamics. For immunomorphological research in a postoperative period standards prepared in respectively with standard methods.
Results. In obedience to the results of immunohistochemical research, growth of myomatous nodes takes place not only due to prevailing of expression of progesterone, but also with participation of estrogen receptors. It is possible to provide for, that in a greater degree on development and growth of uterine fibroids estrogen or progesterone influences not separately, but them complex co-operating with each other. The receptor component of myoma depending on type of clinical growth of uterine fiibroids is different. At fast-growing myoma prevail to and estrogen, and progesterone receptors, on abolition of slow-growing, where enhanceable only to progesterone receptors. Comparison of dynamics of growth and sizes of uterine fiibroids, and also level of expression of receptors of cages of endometrium to the sexual steroid hormones by a statistical analysis on the method of Spirmen found out the following conformity to law: expression of estrogen and progesterone in a uterine fibroids has positive linear cross-correlation dependence on the degree of expression in stroma of endometrium (r =0,6) of р=0,02.
Conclusion. Expression of receptors of estrogen and progesterone has direct linear connection in a myometrium and stroma of endometrium in a the same uterus. The analysis of the conducted researches allowed to come to the conclusion that determination of receptivity of steroid hormones of endometrium can serve as a diagnostic criterion for the estimation of receptive component of uterine fibroids. Findings allow to spread the picture of pathogeny of uterine fibroids for the women of reproductive age and scientifically to ground the necessity of improvement of algorithm of diagnostic and treatment-and-prophylactic measures.
